1095 Form provided by employer or 1095-A if health care was with Covered CA.
If an Employee please bring all W-2 received.
Self employed bring Forms 1099-MISC, Schedule K-1, income records to verify amounts not reported on 1099.
Records of all expenses-check register or credit card statements and receipts-Totals.
Property tax, interest, and insurance forms.
